Customer Review: a review for Brasil '66 fans
It's been around 40 years since Mendes reached an artistic and commercial peak with several classic albums that successfully mixed bossa nova, jazz, and pop music. Those albums still sound fresh today, thanks to the terrific songs, latin rhythms and some sparkling arrangements. Since then, Mendes has experimented with mixing his Brazilian sound with folk rock, funk, dance music, hip hop, and on "Encanto," some r&b. As someone who prefers the work he did with Brasil '66 to anything more recent, to my ears the album is pleasant though generally forgettable with the exception of a few wonderful tracks. Unsurprisingly the best tracks are those with strong melodies by Antonio Carlos Jobim, in particular "Somewhere in the Hills" and "Dreamer," both of which are direct descendants of the sound of Brasil '66. "Dreamer" includes both Herb Alpert and Lani Hall, who sounds remarkably like she did when she was fronting Brasil '66. This is a genuinely stunning track and easily the best on the album. Natalie Cole sings "Somewhere in the Hills" which is appropriately jazzy and is one of Jobim's strongest melodies. Other Jobim tracks include "Waters of March" which Mendes has covered several times in the past. It's a singer's song, and sounds a little rhythmically stiff here, as it has in his previous efforts. "Morning in Rio" is also good, with a more Brazilian styled melody and a nice trombone solo. The rest of the CD is listenable, but not such that I ever want to hear most of it again. "The Look of Love" is an unnecessary remake of a good song. What made the original great was a sensational big band arrangement which has been dropped in favor of a hip hop arrangement. I liked the Brazilian styled funk of "Odo-ya" and the mellow "Catavento" with Mendes' wife on lead vocals. On the other hand, "Funky Bahia" and "Lugar Comum," the latter of which features an Italian rapper doing a little snippet of "Volare," are disposable pop/hip hop songs. If I could get into Mendes' head, I would try to convince him do an album of Jobim and Marcos Valle songs with Herb Alpert and Lani Hall and a small jazz/bossa combo. It might not sell a million copies, but it would be an artistic triumph. If you're an unreconstructed fan of Brasil '66 as I am, I can recommend this with reservations. You might be better off just downloading a handful of tracks. But "Dreamer" should not be missed.
Customer Review: Summery and enchanting.
"Encanto" follows the highly acclaimed 2006 collaboration with will.i.am Timeless and with this latest release, Sergio takes us even deeper into Brazil, having recorded all the basic tracks in Rio and Bahia and finally finishing up in in California, where the studio band included Alphonso Johnson. For "Encanto" (in Spanish for Enchantment), Mendes has also enlisted the formidable and enchantingly diverse talents of an array of guest musicians who hail from the world over. Latin superstar Juanes from Colombia and multi-talented Carlinhos Brown & Vanessa da Mata from Brazil jostle alongside the foremost Japanese pop group Dreams Come True, Belgium's Zap Mama and Italian rapper Jovanotti. This international cast is completed by the American stars Fergie, Siedah Garrett and Herb Alpert who is accompanied by his wife, original Brasil '66 singer, Lani Hall. Old smoothie Mendes gets jiggy on a selection of bossa-favourites with a raft of guest stars including the ubiquitous Will.i.am and Fergie from the Black Eyed Peas, Natalie Cole and Herb Alpert et all... This new album essentially is a cross between Mendes' soft, salsa-tinged melodies and modern day R'n'B. The opening track, "The Look of Love" with rap/vocals by Fergie, is a deliciously moreish funky-fusion, and for the first third or so, the combination of Sergio's Rhodes with the hip-hopped production style is terrific. Perhaps this new version of Burt Bacharach's "The Look Of Love", which Mendes had originally transposed to bossa nova heaven in 1967 is the boldest track on "Encanto". This new interpretation, produced by Black Eyed Peas will.i.am, preserves the alluring melody of the original while bringing the song into the new millennium with crisp drum programming and a sexy rap by Fergie. Then it goes all smooth jazz on you. Not bad, though. Whether you are a Sergio Mendes fan especially the earlier stuff, Brasil 66, Equinox, etc or a Black Eyed Peas fan, or indeed R&B then there's something on this album to please everyone. I'd say for such a challenging album there are only a couple of songs that are just to heavy hip hop for me. The rest is a joy to listen to, if you are open minded about remixes and love to see what Sergio has been up to. This album is a grower. The more I listen to it, the better it gets. You will like it. If ballroom dancing is a past time of yours that you enjoy but you suffer from a verruca, then you will understand the importance of wearing the right shoes. However, even though the right footwear is worn on the feet it is still no guarantee there will be no discomfort. Treatment is needed so that you may continue in upping your performance on the dance floor like that of Mr Gene Kelly. If your dancing has been put on hold because of this problem, then speak with your doctor for advice on how to ease the discomfort so that you can dance again.

A verruca (plantar wart) is a type of wart found on the lower limb (sole of the foot). It is no different to that of a common wart as we know them - but because of the body weight pressing down onto the verruca it then becomes painful and sore. The appearance of this foot wart - is slightly raised and round with a thickened edge of surrounding skin. Other signs and strong points to look out for - to decipher this type of verruca is that of fine tiny black dots. It is not uncommon to find a cluster of them to appear all together.

A mound/clump or clusters of small warts are referred to as 'mosaic warts' - these you will be happy to hear are usually painless. If you are a concerned parent and worried about the children developing warts then do not worry because, this type of verruca in minors - tend to appear and then disappear. I don`t want to burst any ones bubble but warts on adults can persist for a long time (years) if left unattended (not treated).
Just like other warts, a verruca can stem from the strains of the papillomavirus.

Do not be alarmed but papillomavirus is present in huge amounts on a vast majority of different humans which is carried on the surface of their skin. This will not cause any harm or cause problems unless it pierces the skin. Unhealthy and neglected skin e.g. skin with fine cracks on the surface may well be the likely place for this to happen.

There is no real proven guarantee that a verruca is picked up in swimming pools as a great many people are led to believe. In saying that, there is without a doubt plenty of papillomavirus around swimming pools etc, particularly so in the shower area. Remember you are very unlikely to get a verruca if your skin is without flaws. A good idea to minimize any risk if this is a big worry for you is, to consider wearing flip-flops or open sandals in the shower.

If those dancing shoes are left on the shelf for to long - you may just forget how to tango or rumba. Some good advice is to pop along to the doctor so that he/she can check you over. Try not swap shoes.

The Bollywood craze has swept around the world, as fans of "Hindi Cinema" have fallen in love with the kietschy (and yet unusually addictive) high-pitched songs, exaggerated hand and eye movements, and tendency of characters to break into large-scale musical numbers at the oddest moments, and often against anachronistic settings like the Swiss Alps.

But music is an integral part of Bollywood films, so that even a dark and edgy drama has to have at least one catchy song and corresponding dance number woven into the script. In fact, in India, the sound track is released before the movie, so that everyone is humming the song as they enter the theatres.

Most Bollywood songs are actually lip-synched by the actors, the voices "borrowed" from professional playback singers. (The exceptions are Kishore Kumar K. L. Saigal, Suraiyya and Noor Jehan, who are known for both their acting and their singing). were also known as both singers and actors. Amitabh Bachchan, Aamir Khan, Abhishek Bachchan and Shah Rukh Khan also sang in their movies. However, playback singers have their own following-like Lata Mangeshkar, Mohammed Rafi and Yesudas.

Today's Bollywood songs are known for how they remix filmi songs with modern beats and rhythms, adding an interesting edge to the ancient folk dances and steps that make up a typical Bollywood number (although it is not uncommon to see current hits incorporating MTV or Broadway twists).

While some Bollywood songs are directly related to the plot, there are also item numbers where someone performs a catchy number-like a courtesan dancing for a rich client in a cabaret scene, or disco scenes, or dancing at celebrations.

Bollywood soundtracks usually become big hits, and there are cases when the music outsells the movie. Due to their incredible popularity, it is not surprising that many of the songs are now available as ringtones. Now you can listen to "Kabhi kushi kabhi ghum" or " Bolein chooriyaan" on your phone. Some come in polyphonic form, but many are already available as true tones.

Customer Review: Pleasant "Quickie" Read of How Coldplay Broke the US
I'm a big Coldplay fan, and when I saw this book at my local library, I just picked it up out of curiosity. The author, Gary Spivack, was a record label executive at Capitol (he's since then departed) and responsible for marketing/supporting Coldplay in the US. "Coldplay: Look At the Stars" is Spivack's personal take on how Coldplay broke the US market, and in that it succeeds. He obviously had a lot of contact with the band, and that is where the book succeeds. My favorite part of the book is Gary's recollections on Coldplay's first American tour in February, 2001, for no other reason than I happened to see Coldplay in Portland, OR (the second stop in the US). You could just feel the magic in the air, a band that was about to make it really big. The author doesn't spend a lot of time on how the band actually formed and where it found its musical origins, and that is the major failing of this book. Nevertheless, this is a quick and pleasant read. Spivack recounts his sacking from Capitol in early 2003, and how quick Chris Martin was there to console him (by phone). But for the definitive Coldplay story, we will have to wait another day and another book.
Customer Review: "Look at the record exec."
I'm a musician and a big Coldplay fan, so I should have loved this book, but it fell short. This book read more like a tour book you'd buy at a concert put together by their marketing people than an insider's story. The main problem I had is it was written by a record executive who claims to be "insider", but has nothing new to offer. I suppose Gary's perspective would be interesting to fellow record executives and the like, but the book falls short in revealing any real insight on the band that you couldn't read in a magazine interview with the band. One of Gary's "insights" is how humble Chris Martin is...he quotes Chris in a conversation early in the book where Chris asks Gary "how can I make your job easier" and Gary beams. WHO CARES. Chris Martin is humble and selfless to a fault; no insight here. The problem is most of Gary's interactions with the band are interactions between a record exec. and the band...and it reads that way. The fact is, Gary had very little to do with Coldplay's success and it's obvious he is not much of an insider. Gary admits himself Coldplay was "lightning in a bottle", signed only after a fierce bidding war between labels. I think Chris Martin is just a really nice guy, who saw someone (Gary Spivack) down on his luck and wanted to help the poor guy out by letting him write this book (Gary had been fired by Columbia just before penning the book). The author seems to blame his lack of an insightful story on Coldplay's middleclass upbringing and that they're all around good kids (this isn't a drugs, sex and rock and roll type band). I don't agree that an interesting story couldn't be written by the right person. My biggest problem is this book is written by a record company exec. who approached the project like you would a tour guide sold at a concert venue, only longer. Gary didn't grow up with the band (to the point they were signed), didn't discover the band (they were signed by a sister co. in England), doesn't manage the band (where he'd be touring every night seeing all sides of the members), or produce the band (where he's in the studio witnessing the creavtive process)...he just booked tours and showed up occasionally for concerts and "meet and greets". How does that qualify you to write a band biography? Coldplay should have their manager, producer or the members themselves write this story...with a side of the story to tell that we don't already know.

Typically, a wedding lasts about 15 minutes. However, some guests do arrive early to the reception. Therefore, be sure all reception to-dos are complete by the start time of the wedding. Also, all wedding vendors should be wearing proper attire for early arriving and late staying guests. All tables should be set-up including cake table, entertainments table, sign-in table, food tables, and tables with chairs for all guests. If you have a seating arrangement, all seating lists should be at the reception with everyones name with where they are sitting.

Introduction of wedding party

This step is not mandatory, but it is nice for everyone to know who the wedding party is as many have not met before your wedding. Create a Reception Planning Guide and give this to your master of ceremonies prior to your wedding. This details, among other things, the order of entrance into the reception site with their names and titles. The order of entrance is as follows: parents of the bride, parents of the groom, ushers with bridesmaids, flower girl and ring bearer, special guests, best man, maid/matron of honor, bride and groom. In addition, review the pronunciations of the wedding party's names with the master of ceremonies.

Introduction of bride and groom

This is always the last of the introductions. Everyone should stand before the bride and groom enter. In addition, a special song can be arranged with the music entertainment and a special announcement made to punctuate a true grand entrance. Also, inform the master of ceremonies how you would like to be introduced: Mr. and Mrs. Smith or John and Jane Doe.

Blessing

This is another step that is not mandatory, but for the religious couples makes a nice setting. If you invite the person who performed the marriage ceremony to the reception, I would recommend having them conduct the blessing. If they are not able to attend, a parent or family friend is a good idea. Lastly, the master of ceremonies could give the meal blessing. Be sure to communicate with whomever is giving the blessing in advance so they are prepared to give a personal touch.

Meal

It's time to eat! Bride and Groom, be sure to stop and take a moment to eat. This may be your only chance in the day. In addition, it is customary for the bride and groom to start the food line. Most guests know to wait until the bride and groom start the food line. So don't be late!

Cake Cutting

This is the traditional bride and groom making the first cut on their wedding cake. Then, the bride feeds half of the piece of cake to the groom and the groom feeds the remaining half to the bride. The cutting of the cake is a ceremony intended to symbolize the caring and sharing for one another. Forks may be used as they make the image very attractive for the photographer. Forks may also minimize the possibility of someone being hurt with the more playful squashing that sometimes happens.

Toasts

The toast is when the bride and groom toast each other then interlock arms and drink. Immediately following, the best man and maid/matron of honor make toasts to the bridal couple. Be prepared for other family and friends to follow with their toasts too.

Bride & Grooms First Dance

The Bride & Grooms Dance is the first dance between a bride and groom as a married couple. The dance is also commonly known as the "First Dance". The timing of the bride groom dance can vary in the evening based on your preferences. The first dance can be done immediately following the grand entrance with the wedding party circling the dance.

Father & Daughters Dance

The Father & Daughters Dance or the Father Bride Dance is the dance between the father(s) of the bride and the bride. Brides, if you have more than one father in your life, one can tap the other on the shoulder in the middle of the dance so you can dance with both of them. If you do not have a father, a common substitute is a father figure or even your brother would make a very nice gesture.

Mother & Grooms Dance

The Mother & Grooms dance is the dance between the mother(s) of the groom and the groom. Grooms, if you have more than one mother in your life, one can tap the other on the shoulder in the middle of the dance so you can dance with both of them. If you do not have a mother, a common substitute is a mother figure or even your sister would make a very nice gesture.

Special Dances

There may be songs in your life that mean dear to your heart. You can ask the music entertainment to play these songs during the night or immediately following the formal dances. An example would be: If someone dear to you passed recently, you may ask the music entertainment to play Angels Among Us by Alabama.

Bouquet and Garter Toss

The traditional tossing of the bouquet is when the bride tosses the bridal bouquet to all single women in attendance. Immediately following the bouquet tossing, a chair is set in the middle of the dance floor. The chair is for the bride to sit and the groom to remove the bride's garter from her leg. The traditional tossing of the garter is when the groom tosses the bride's garter to all the single men in attendance. The people whom catch the garter and bouquet are said to be the next one to marry.

Dancing and Fun

This is what you pay the music entertainment to do...get people to dance and have a good time. Give the music entertainment music examples and limitations of what type of music you want but dont give them a list of 100 songs they need to play.

IVR is short for Interactive Voice Response, an automated system that uses a touch response telephone to direct your call. You will have one of these if you have multiple departments with lots of incoming calls.

60% of callers put on a silent on-hold hang up within 40 seconds and callers put on-hold with information and music will stay about 3 minutes longer.

55% of communication is lost over the phone (body language), leaving us with 38% through tone of voice and just 7% through words.

As you can see from the statistics, giving the wrong message in the wrong tone could actually damage your sales and your reputation.

Many companies spend time and money on their visual branding (logo and leaflets) but neglect their telephone audio. "Your sonic branding is as important as your visual branding!"

This article looks at some of the common mistakes made with IVR and on-hold marketing. It also outlines some good techniques to use.

You Rang!

It is best to have just a few options on your telephone system. People lose interest quickly so listing every department is not always a good idea. Think about it from the customers' point of view, what will they be calling your company for? To buy products? Information? To make a complaint?

It is recommended that you keep it to 3 or 4 salient options max if you can. If you think that you need 7 or 8 options, be ruthless; cut it down to the 3 or 4 relevant points. You can always have the 4th option to cover all other enquires.

Remember, the purpose of telephone options is to get the potential client through to the most relevant operator as quickly as possible. Failing that, you need to get them to remain on hold until you are able to deal with their call.

"Thank you for calling ABC, for sales please press 1 - for product information press 2 - for all other enquiries press 3."

Make it simple and friendly and always use a professional voice-over. People relate to it better and psychologically, it is more accepted and expected by the caller. Your company will sound established and efficient and will instantly inspire confidence. You can always tell when a member of the office records the IVR options, you know, the one with the best telephone voice!

Think about the type of voice-over you want to use... What image do you want your company to portray in the mind of the listener? What is your target market?

Using a voice-over style that will connect with your caller is important. You don't want a mature corporate voice if your company is aimed at 18-25 year olds, and likewise, you don't want a young radio type voice if your are a firm of lawyers.

On-hold Now

Being on hold can be annoying, however, you can entertain and use this time to educate your caller. A silent on-hold is extremely cold and callers hang up much sooner than when they have something to occupy them.

Music

Music can alleviate boredom, however, make sure your music is properly licensed for use on a telephone system otherwise you could face a copyright infringement charge.

Also, do select the music carefully. Not everybody has your taste in music...for example, a banging hip-hop track may alienate some of your callers. It is usually best to go for music that is easy on the ear or relaxing. Original music is a good choice for telephone use as the caller has no preconceptions about the piece - they just accept it as music.

As with the voice, it is also advisable to pick music that is relevant to the company image you want to portray. It is acceptable for a nightclub to use a pumping dance track, but clearly this would be wrong for an accountant's office.

It is good practice to thank the caller for holding, but do not do it every 5 seconds! Many on hold systems work on a continuous loop. I suggest having your thank you' message at the beginning and in the middle of the loop. Intervals of 20-30 seconds don't disrupt the music too much but remind your caller that you care.

Entertain, educate and sell!

Don't waste the opportunity you have when a caller is on hold. You have a captive audience. You have a great opportunity to keep them occupied and educate them about your services.

It is best not to blatantly advertise, or to cram as many ads in as possible. A more subtle approach is far less intrusive and more appreciated by the caller. On-hold marketing should use voice and music combined. They are both important elements so let any adverts breath; make sure there is music on either side of them to break things up.

When it comes to writing a telephone advert, make it short and to the point. I am not and advertising expert, however, making it engaging and entertaining can really count.

Simple stuff can work best i.e.

"Thank you for holding, did you know ABC limited sells more widgets in the UK than any other company, and can price match any quote."

"This Autumn we have great savings on widgets, ask your operator for details".

You get the idea, short sentences, no hard sell and let people hear the music before and after the voice, this is when they will take in and process the information.

I have written this article based on my experience of putting telephone audio packages together for clients and have much experience in this area. I have a limited knowledge about hardware but I advise that you always test any newly installed audio.

Phone your company as a client to check that it all works. Is the volume right? Is it clear? Are there anomalies in the system that causes delays or the first split second to be cut off? There is nothing worst than hearing your perfectly produced audio file being butchered by incorrectly configured hardware. Your callers will notice it but say nothing about it. Not to you anyway!

I don't consider my house a home without this album. I seriously put it on repeat. It's one of my favorite to cook and clean to and just leave in the stereo when guests arrive. The DVD of the making of this CD provides a valuable context to what these musicians had been through and make the experience of the album that much more potent for me--I'd recommend the DVD, as well. Ry Cooder does a great job with both.

Customer Review: a review for Brasil '66 fans
It's been around 40 years since Mendes reached an artistic and commercial peak with several classic albums that successfully mixed bossa nova, jazz, and pop music. Those albums still sound fresh today, thanks to the terrific songs, latin rhythms and some sparkling arrangements. Since then, Mendes has experimented with mixing his Brazilian sound with folk rock, funk, dance music, hip hop, and on "Encanto," some r&b. As someone who prefers the work he did with Brasil '66 to anything more recent, to my ears the album is pleasant though generally forgettable with the exception of a few wonderful tracks. Unsurprisingly the best tracks are those with strong melodies by Antonio Carlos Jobim, in particular "Somewhere in the Hills" and "Dreamer," both of which are direct descendants of the sound of Brasil '66. "Dreamer" includes both Herb Alpert and Lani Hall, who sounds remarkably like she did when she was fronting Brasil '66. This is a genuinely stunning track and easily the best on the album. Natalie Cole sings "Somewhere in the Hills" which is appropriately jazzy and is one of Jobim's strongest melodies. Other Jobim tracks include "Waters of March" which Mendes has covered several times in the past. It's a singer's song, and sounds a little rhythmically stiff here, as it has in his previous efforts. "Morning in Rio" is also good, with a more Brazilian styled melody and a nice trombone solo. The rest of the CD is listenable, but not such that I ever want to hear most of it again. "The Look of Love" is an unnecessary remake of a good song. What made the original great was a sensational big band arrangement which has been dropped in favor of a hip hop arrangement. I liked the Brazilian styled funk of "Odo-ya" and the mellow "Catavento" with Mendes' wife on lead vocals. On the other hand, "Funky Bahia" and "Lugar Comum," the latter of which features an Italian rapper doing a little snippet of "Volare," are disposable pop/hip hop songs. If I could get into Mendes' head, I would try to convince him do an album of Jobim and Marcos Valle songs with Herb Alpert and Lani Hall and a small jazz/bossa combo. It might not sell a million copies, but it would be an artistic triumph. If you're an unreconstructed fan of Brasil '66 as I am, I can recommend this with reservations. You might be better off just downloading a handful of tracks. But "Dreamer" should not be missed.

"Encanto" follows the highly acclaimed 2006 collaboration with will.i.am Timeless and with this latest release, Sergio takes us even deeper into Brazil, having recorded all the basic tracks in Rio and Bahia and finally finishing up in in California, where the studio band included Alphonso Johnson. For "Encanto" (in Spanish for Enchantment), Mendes has also enlisted the formidable and enchantingly diverse talents of an array of guest musicians who hail from the world over. Latin superstar Juanes from Colombia and multi-talented Carlinhos Brown & Vanessa da Mata from Brazil jostle alongside the foremost Japanese pop group Dreams Come True, Belgium's Zap Mama and Italian rapper Jovanotti. This international cast is completed by the American stars Fergie, Siedah Garrett and Herb Alpert who is accompanied by his wife, original Brasil '66 singer, Lani Hall. Old smoothie Mendes gets jiggy on a selection of bossa-favourites with a raft of guest stars including the ubiquitous Will.i.am and Fergie from the Black Eyed Peas, Natalie Cole and Herb Alpert et all... This new album essentially is a cross between Mendes' soft, salsa-tinged melodies and modern day R'n'B. The opening track, "The Look of Love" with rap/vocals by Fergie, is a deliciously moreish funky-fusion, and for the first third or so, the combination of Sergio's Rhodes with the hip-hopped production style is terrific. Perhaps this new version of Burt Bacharach's "The Look Of Love", which Mendes had originally transposed to bossa nova heaven in 1967 is the boldest track on "Encanto". This new interpretation, produced by Black Eyed Peas will.i.am, preserves the alluring melody of the original while bringing the song into the new millennium with crisp drum programming and a sexy rap by Fergie. Then it goes all smooth jazz on you. Not bad, though. Whether you are a Sergio Mendes fan especially the earlier stuff, Brasil 66, Equinox, etc or a Black Eyed Peas fan, or indeed R&B then there's something on this album to please everyone. I'd say for such a challenging album there are only a couple of songs that are just to heavy hip hop for me. The rest is a joy to listen to, if you are open minded about remixes and love to see what Sergio has been up to. This album is a grower. The more I listen to it, the better it gets. You will like it. Picks of the album: "The Look of Love", "Waters Of March", "Dreamer", and "Somewhere In The Hills (O Morro Nao Tem Vez)". Like many other internet users, I was pretty skeptical about visiting a download movie site. I've heard nightmare stories of people who download movie, new computer virus comes with it! Short answer: anything can become infected. Though I do no think ruptured ovarian cyst becomes infected very often, did not hear about that. I will look more literature and probably place the answer on my website.

I didn't do very extensive literature search. Should admit.
Anyway, some available books mention that ovarian cyst may become infected. However the infection is not described as the main complication in ovarian cyst rupture.

Also, I don't remember that anybody told me otherwise. Maybe there is some specialized article that says: the condition happens in one point three percent of cases with Standard Deviation of half percent. I don't know exact percentage. Need to look more. PubMed service did not give many abstracts on PCOS + infection.

Anyway.

So how would it look alike?

A young woman comes to ER. She is premenopausal. She complains on mild (or maybe severe) pain in her belly. ER Doctor takes history. The woman also mentions changes in her menstrual interval. Let's say regular is 28 days. Last one was delayed.

Physician puts gloves, puts jelly on gloves. Then he puts his two fingers into the female vagina.
The other hand is on belly. Then he starts to palpate.

It is named pelvic exam. Modest name. Though in Russia it is named vaginal exam, which it is.

Is it a common type of exam? Depends. They usually send you to CT (computer tomography) scan if there is severe abdominal pain. Charge 1000. Boom.
Done.

Exclude the price. Exclude delay in reading (somebody should look and interpret what is going on). Exclude radiation. CT scan gives better picture than just poking your belly.

CT scan helps to diagnose abdominal pain of uncertain origin. You can really image what is going on. Though, there are cases when physical exam gives more clues. Physical exam must be performed always. Pelvic exam is somewhat a special one.

I remember how I performed a pelvic exam in medical school. It is actually difficult even just to insert two fingers into vagina first time.
Female Gynecologist asks me: "So, what do you feel?"
Patient goes the same, encourages me:
"What do you feel, what do you feel, do you feel it?"

I guess she felt a sort of museum artifact.
Heck, I did not feel anything.

Well. Actually I felt something - aside from uterus - something round. I would say 5 cm in diameter (would it be less I probably would not feel it at all) and semi-solid on touch. Also I saw that the patient grimaces. It is tender when I push hard.

It's it. How to say that it was tuboovarian abscess (that it was) for sure, I dont know.
You really need experience to perform this type of exam. Experienced gynecologist can tell almost precisely what is going on.

Let's discuss that woman in ER. She will have tenderness on one side. Physician should be able to feel a mobile cystic mass.
(Cyst or rather cystis is Latin for bubble. Palpate is Latin for touch. It means you touch something and feel what it is).
What if the pain is severe? It often means that the cyst ruptured.
My impression is that modern ER orders CT scan right away. If you are not very sure what is going on, you will go from less expensive methods to more expensive and end up with CT anyway. Ruptured cyst causes significant pain. Here CT is indicated.

Alternatively they may order Ultrasound Exam. Transvaginal ultrasound uses the probe inserted into vagina. Ultrasound is cheaper than CT. Ultrasound visualizes cysts clearly. Though, ultrasound gives less information for excluding other pathology. Ultrasound is also safe from the radiation point of view.

In PCOS ultrasound shows increased number of small cysts in both ovaries. Usually more than five confirms the diagnosis.

Culdocentesis may give some useful information too. The name came from cul-de-sac. It's French I guess. Cul-de-sac is one of the pouches in the pelvis. Centesis means: stick a needle and draw. These days it is considered an outdated method. But if you do not have other machines, it is very useful.

If the content is blood, the ruptured cyst was probably Corpus luteum cyst. If the content is purulent the ruptured thing was probably a tubo-ovarian abscess or other pelvic inflammatory disease (PID).
Other abnormal masses can rupture as well. Teratoma gives oily fluid, endometrioma gives "chocolate" old blood.

What is a follicle?

Female body is created for reproduction and childbearing. Oocyte is the start for a new human being in the ovaries. Several layers of specialized membranes surround an oocyte.

The membranes protect the oocyte, help in feeding and nurturing of this small cell.
One of layers has a beautiful name Zona pellucida. Pellucida means shiny in Latin.

When the oocyte matures, a small bubble (follicle) filled with special fluid is formed around.
In mid-cycle the follicle bursts and the oocyte goes first into peritoneal cavity, next into ovarian tubes (fallopian tubes). The tubes lead into uterus. Tubes, by the way, have special small hair-like things inside - fimbria. They beat in one direction. They propel the oocyte into uterus.

I remember I read somewhere that there are 11000 follicles. When a girl is born, there is no more multiplication of oocytes. After the birth the follicles sit dormant. When the female goes into her reproductive age, the follicles start to grow and mature (one by one).

Only 400 of them mature.

Yeah, it should be like this. Calculate. Average cycle is 28 days. So there are around 12 cycles a year. Women start to menstruate at 13-15 years old. The menopause is around 45-55 years. Total is 30-40 years

Multiply everything together. It should be around 400.

By the way, an interesting thought.

Somewhere in nineteen century the baby was considered the baby when it was born. The church even struggled to admit anything like existence of cells etc. Rare baby actually survived beyond first year.
Heck, the hypothesis that human been consists of small cells was actually admitted widely not so long ago. Maybe hundred years ago. Then, all that research happened. People learned how the fetus is created and how it grows. Now the public idea is that fertilized oocyte is already the baby.

Have you seen any oocyte under microscope? Even a human hair near an oocyte looks like a skyscraper near a real human.

Now, if the public perception had shifted this way in several decades, shouldn't we punish all women for that they recklessly loose 400 potential babies during lifetime. Isn't it a crime?

Then, maybe we should punish every man for losing millions of sperms - also potential babies.
Where did this idea come from that fertilized oocyte is the baby and non-fertilized oocyte is not?
Shouldn't we move the boundary a little bit earlier?
Need to think about that.

Anyway.

Ovarian follicle (follicle means small bubble in Latin) usually mature, rupture and release the oocyte that was in this follicle. Sometime the rupture delays. Then ovulation delayes. (Ovulation is rupture and release of the oocyte. Oocyte is the cell that eventually becomes the fetus after sperm gives the genetic material).

Normal cycle is divided into follicular phase (when the follicle grows) and luteal phase.
Luteum means yellow in Latin.

When the follicle ruptures (by the way rupture means burst or tearing), the oocyte goes out.

The cavity that left behind (remember it was small bubble) is filled with blood and special cells, producing hormones. These special cells grow in quantity and fill that cavity. These cells produce hormones that help the fertilized oocyte to attach and to grow in the uterus. Because they grow in quantity, they create a yellowish body in the ovary. It is literally yellowish. The name is Corpus Luteum (corpus=body, luteum = yellow).

This is normal cycle.

As we said, the follicle sometime doesnt rupture (there is a bunch of reasons). A physician should sort out several different conditions. This is an abnormal cycle.
If follicle does not rupture it becomes the follicular cyst. Cyst also means bubble in Latin. There are actually plenty of different kinds of bubbles in medical Latin. Big ones and small ones. Normal and abnormal.

OK, the cyst did not rupture. Then what happens?

Well. If cyst doesn't rupture, it usually resolves. That fluid inside the cyst is reabsorbed and the cyst collapses.

However, if the cyst ruptures, it causes acute pain. The pain comes from irritation of peritoneum (lining of peritoneal cavity) with blood and cyst content.

Why it is not painful when a regular follicle ruptures and releases the oocyte? Probably, a regular follicle is too small. In addition it doesn't cause much bleeding.

In contrast the cyst is a really big bubble (sometime 5-10 cm in diameter). If it ruptures, it instantaneously release bunch of special fluid. Plus, there is significant bleeding because there are a lot of blood vessels around to feed.

Significant is of course relative.

For example, take 5-10-20 ml of blood from a patient vein in a hospital daily. He complains about the pain from the needle mostly.

But if you get the same 10 ml of blood into peritoneum... Wow.
You will cry. There are plenty of nerve endings. Peritoneum is too touchy-feely. Tender.

Besides, the cyst has high concentration of prostaglandins. Prostaglandins, in their turn, are mediators of inflammation. They should cause significant pain directly and indirectly.

From the other hand bleeding could be really significant. Then it becomes really dangerous.

A physician also should not miss an ectopic pregnancy. Doctor will order a pregnancy test for that. If an ectopic pregnancy starts to bleed, this is really really worrisome. It seems like your blood did not left your body. However the blood is in the abdominal cavity. It left the blood vessels. It is internal bleeding. You die quickly.

Polycystic ovarian syndrome is a little bit different animal actually. Here is some genetic predisposition.

Classically: an overweight young female presents with oligomenorrhea or amenorrhea, anovulation, acne, hirsutism, and or infertility.

What is what? Poly = many. Many, many, many men.
So PCOS means bunch of those bubbles in the ovaries. The follicles did not rupture on time, as they should. Oligo means a little. Meno is derived from menses. Rrhea means flow in Latin

So olygomenorrhea = flowing a little bit (less than it should).
A- is a prefix that means "No". So, amenorrhea = no flow at all.
Hirsutism. I don't remember where it came from, but means hairy or hairiness. Actually excessive hairiness.

Causes of PCOD or PCOS (disease or syndrome) are obesity, genetic predisposition and some other causes of Luteinizing hormone (LH) excess.

There is a self-amplifying cycle:

LH stimulates theca lutein cells. Theca means sort of capsule. Doesn't really matter, just an anatomical term.
Those cells are special. They produce androstendione and testosterone. Androstendione and testosterone are actually male hormones. You know, bodybuilders use these hormones to get muscle bulk. You probably heard about those hormones. Sport doping uses testosterone.
So, athletes build their muscles and trash their liver.

Rumors say that a famous Hollywood actor used the hormones. Later he got liver transplant.
Though he always denied the use.

Anyway, female body converts androstendione into estrone (a weak estrogen). Fat cells do this. Estrone is a female hormone already.

Basically any body produces androgens (andros = man) and estrogens (female hormones). Just the proportion of those hormones makes us male or female.

The cycle happens in normal person as well.

The estrone stimulates pituitary secretion of LH.
Pituitary is a small gland in you brain. Pea Size.
It's small, but it sooooo powerful.

Pituitary has another name - hypophysis. Hypo means down, phys means growth, so this gland is growing from below the rest of the brain. Pituitary gets bunch of connections from hypothalamus.
Hypothalamus means below thalamus.
These two areas of brain regulate almost all the hormone production in organism.

Higher levels in brain hierarchy regulate them.

Hypophysis gets a command. Then it produces some intermediate messengers and hormones.
The hormones go into blood and control whole body.

Hormones are like orders, like messages to the rest of the body.

Brain may give quick orders: Signals go through the nerves. It is like a phone order or cablegram.

Brain also regulates organism through the hormones. This is like a mail order.
Sort of if the brain sends letters by regular mail. The hypophysis is the Post Office in this case.

PCOS kicks in when a woman is obese. There are more fat cells to convert
androstendione to estrone. Estrone has such effect that it stimulates pituitary secretion of LH.
LH in its turn goes back to those theca lutein cells we discussed and turns them on again, to produce more androstendione, which is again converted into estrone.

Self-amplifying cycle

In addition, that increased level of testosterone causes the hirsutism (she becomes hairy like a male) and acne in female.
In a normal person this cycle is probably designed to support the development of fetus.
Estrogen helps placenta to grow. Placenta supports fetal growth.

However, in a person with PCOD the cycle is going out of normal control. In this case LH causes growth of the cysts in the ovaries.

Why?

Because the corpus luteum cyst is partially made by overgrowth of those theca lutein cells. LH stimulates theca lutein cells.

Also, women with PCOS have intolerance to glucose (sugar) and resistance to insulin.
It means there is a lot of insulin (hormone that helps to utilize glucose mainly).

However excessive insulin does not work. Either receptors to insulin do not work or something else, but the glucose is not utilized. Hence, energy inside the cells drops. Hence, a big pile of other problems mounts. As if it is Diabetes Mellitus. Diabetes is a different topic of discussion. For us, it is worthwhile to mention that people with diabetes are very much prone to any infection.

PCOS causes acanthosis nigricans also. Acantocytes are special skin cells.
Nigricans means black in Latin. That thing looks like thickened pigmented skin. When you touch it, it feels like velvet. Usually it happens in axilla, neck, below breast, in inner thigh and vulva. So, mostly all those places where skin folds.

The treatment for PCOS includes different medications: oral contraceptives, progesterone,
glucocorticoids, ketoconazole, spironolactone, cyproterone, flutamide, cimetidine, finasteride, ovarian wedge resection, laparascopic electoracutery, mechanical hair removal, etc.

All methods break the cycle of overproduction. The medications are either hormones themself or hormone-like substances that occupy receptor site and prevent regular hormone to work.

The medications act on different levels. Normal hormones have very complicated regulation. There are loops and feedbacks in the pathways.

To suppress a hormone production or action, you give similar hormone or another hormone or non-hormone at all, that goes to the feedback loop and breaks it and so on. It's really long separate discussion.

Basically, you either decrease hormone production or shift ratio toward female hormones.

Another way, the best probably, is weight loss. No fat cells - no conversion of andrgoens etc You can make conclusions yourself.
It's the first line of treatment.

For a simple follicular ovarian cyst (not PCOS) doctor rules out ectopic pregnancy. Then he may send patient home and repeat pelvic exam in 6-8 weeks. Especially, if the cyst was small, less than five cm in diameter.

For larger cysts, doctor would order pelvic ultrasound.

Most follicular cyst will resolve on their own in six to eight weeks. Though, a physician may give oral contraceptives. Again, this suppresses stimulation of cyst by hormones from the hypophysis. The hormones are named gonadotropins.

If the cyst is still there after 6-8 weeks, a suspicion arises that the cyst maybe malignant. Then doctor orders other studies. CT scan. Physician may perform surgical procedures also. He looks what is this cyst really.

Corpus luteum cyst is usually not treated. However, oral contraceptives may be used.

Rupture of any kind of those cysts leads to another story. Acute pain, bleeding into peritoneum.
Sometime bleeding is very severe and is true emergency. You need also to distinguish other process in the abdomen. For example, appendicitis looks similar. You can treat mild case of non-complicated cyst rupture with just observation. Appendicitis almost always requires surgery.

There are many other problems arise. Surgeon scratches his head: what's going on? Is this this or is this that? Here is the CT scan gives big advantage.

Now, going back to the question of Ms. L.

If the cyst was infected, I don't' see a reason why a ruptured cyst wouldn't become infected.
Cyst content is very nutrient-rich. Remember? All those cells and their products are dedicated to feeding the oocyte (future baby). Should be very tasty for any bacteria.

Rupture may cause significant bleeding as well. This blood is also different from the blood in your vessels.

This blood is sitting in the pelvis, not moving, quickly clotting. Clotting prevents entry of white blood cells. "No flow" prevents entry of antibodies. Absence of flow prevents entry of other protective chemicals (complement etc).

So, it is very nutrient-rich media for bacteria growth.

They can go wild. Why not?
If a female had another pelvic infection before, that infection can flare up. In a normal person peritoneal cavity should be sterile. However, any gynecological or gastrointestinal infection may supply bacteria. Now, mix these bacteria with the content of the leaking cyst. It just destined to become infected.

Actually Ms. L later answered her own question in another e-mail. She had cysts multiple times and they became infected several times.

So, to answer the question:
Will the ruptured cyst become infected? Not necessarily. Rather not. Can it become infected?
Yes.

6/21/08 Before you read this review check out Coldplay's latest CD "Viva la Vida". It is light years ahead of this CD "X&Y". I guess I am so addicted to Pink Floyd, Led Zeppelin, Deep Purple, and other great bands of the 70s-80s that Coldplay falls flat on it's face for me. Or, my expectations of "almost uniformly" stellar British rock bands is unrealistic. As most people do when listening to music, one finds that you do compare the new with the old. And so, in fairness to the change in technology and my own personal taste, I compared this band to my current favorite in world rock music, the astonishing rock-your-rear-off Afro Celt Sound System Band whose singer Iarla O'Liondird's vocal range is so utterly astounding. (Even Josh Groban plays AFCSS at his live concerts). After listening four times to this CD "X&Y", I still found Chris Martin's nasal, and at times, irritatingly whiney voice boring and monotonously repetitive. He is up and down his vocal range over and over on nearly every song. About midway through the CD it felt like I was listening to the same song over and over with musical variations. For me the song lyrics were mostly juvenile and often trite. Especially on the song "X&Y". HOWEVER, Their band members' musicality and instrument talent is undeniable. For this reason I can say I somewhat enjoyed the CD. Overall, if I have to pick a listen to again, maybe, these songs merit a redux: "White Shadows"; "A Message"; "Low".
